12/11/14 <br /> Elk River ADDITIONAL RESIDENTIAL ELECTRIC <br /> Municipal Utilities SERVICE AGREEMENT <br /> The Elk River Municipal Utilities(ERMU)does hereby agree to install secondary electric service lines for the purpose <br /> of furnishing electric service to residential dwellings owned by: <br /> Owner <br /> Mailing Address <br /> Phone# Fax# Email <br /> (Hereinafter called the Owner),in accordance with the following terms and conditions: <br /> 1. Location of premises: Service Size: Amps <br /> Address <br /> Lot Block Addition <br /> 2. Installation shall commence upon receipt by ERMU from the Owner of the following: <br /> A) Property has been graded to within 4"of final grade. <br /> B) Obstructions and debris have been removed from the route of the service. <br /> C) Notice that the installation of the electrical service panel is completed. By-Pass meter socket location shall <br /> be installed on the exterior of the building suitable for radio read,WITHIN 10 FEET OF THE CLOSEST CORNER <br /> OF BUILDING FROM THE POINT OF ELECTRIC SERVICE as determined by ERMU and located AT A POINT(4%) <br /> FEET ABOVE GRADE. <br /> Service entrance shall be located as one of the following: <br /> • Closest corner to the ERMU electric source. ERMU furnishes,installs,and maintains secondary conductor. <br /> • At service pedestal on meter post. Owner furnishes,installs,and maintains secondary conductor from <br /> service pedestal. <br /> D) Inspection sheet from a licensed electrician or inspection card from the State Electrical Inspector. <br /> E) Must prepay connection fee per ERMU fee schedule,plus tax. <br /> 3. The prepaid connection fee,per ERMU fee schedule,is the minimum amount for an additional electric service. <br /> Any additional cost incurred will be charged to Owner at 100%of ERMU cost. <br /> 4. The utilities will provide underground service ONLY when a trench is provided by others. Trench shall be a <br /> minimum of 30"deep and 3"wide. <br /> 5. The electric line installed by the Utilities shall remain the property of ERMU and the payment by the Owner shall <br /> not entitle Owner to ownership interest or rights therein. <br /> 6. ERMU shall not be responsible for delays in making installation caused by inability to obtain materials,strikes or <br /> other unusual conditions not under its control. <br /> By signing below,Owners confirms that Owner has read,understands,and agrees to the terms and conditions set <br /> forth above. <br /> Owner Date <br /> 118 <br />
