AdmissionCriteria <br />AdmissionCriteria:Individualsconsideredforplacementmusthaveadiagnosisofaserious <br />andpersistentmental illnessby aMental HealthProfessional.Noindividualisdiscriminated <br />againstinadmission, provisionofservices,orterminationbecauseofrace, color,national <br />origin,religion, gender,physicalhandicap, sexualpreference,maritalstatus,orpublic <br />assistancestatus. <br />Applicantsseekingadmissiontotheresidential treatment/rehabilitationprogramwillmeet <br />followingcriteria: someorallthe <br />1) IndividualhasamentalillnessasdefinedincomprehensivementalhealthactMINN. <br />Statues245.461. Subd.20.(a): <br />2) Those onaprovisional dischargeorstayofcommitmentwhohavethe potential ofself-harm <br />maybeappropriateforthislevelofserviceiftheprogramdeterminesthisserviceprovides <br />adequatestructureandtherearesufficientsupport systemsinplaceinthecommunityand <br />safetyofothersisconsidered; <br />3)Individualcanreasonablybeexpectedtocommenceorresumeillnessmanagementandrecovery <br />skills/strategiesatthislevelofserviceandneeds24-hoursupervisedandmonitoredandfocused <br />treatment approachtoaccomplishthisimprovementinfunctioningandavoidrelapserequiringa <br />higherlevelofcare; <br />Individualsymptomsaredifficult tomanage,orclearlyinappropriatefor,activetreatmentsatalesser5) <br />levelofcare; <br />6) Individualneedsarestrictivesettingandisatriskofsignificantfunctionaldeteriorationifintensive <br />residential treatmentservicesarenotreceived. <br />7) Individualhasoneormoreofthefollowing: <br />Ahistoryreoccurring orprolongedinpatienthospitalizationsinthepastyear <br />Significantindependentlivinginstability <br />Homelessness <br />Veryfrequentuse ofmentalhealthandrelatedservicesyieldingpooroutcomesinoutpatient <br />communitysupporttreatment <br />8) Individual,inthewrittenopinionofalicensedmentalhealthprofessionalandin consultationwith <br />thementalhealthcasemanager,hastheneedformentalhealthservicesthatcannot bemet with <br />ortheindividualislikelytoexperience amentalhealthotheravailablecommunity-basedservices <br />crisisorrequireamorerestrictivesettingifintensive rehabilitativementalhealthservicesarenot <br />provided <br />9) Applicantsmustnotneedspecializeddietand/ornursingcare. <br />10)Reviewofstaffingpatternsandcompetenciesofstaffresultinadeterminationthattheprogramcan <br />meettheneedsofapersonseekingadmission <br />OtherDiagnostic CategoriesandClassificationsweWillAcceptthatareSecondary toMentalIllness <br />1)Wewill takeclientswhohaveasecondary diagnosisofchemicaldependency aslongasthey <br />are appropriate totheprogram. <br />2)Wehavetakenpeoplewho haveasecondarydiagnosis ofdifferent personalitydisorders <br />depending on theclient mixandourabilitytoworkwiththem <br />3)Wemay takesomepeoplewho haveamild TBIsecondarytomentalillnessdepending the <br />client’sabilitytogainfromtheprogram. <br />4)AccordingtopolicyattheMinnesota DepartmentofHumanServiceswecannottakelevel3 <br />sexoffenders. Wemay takealevel1or2ifwebelievethe personwill benefitfromthe <br />program andhavea history offunctioning wellinthecommunity. Wetrytoconsider thecurrent <br />mixofclients andthecommunity theylivein. Itisimportant tonotethatmentalIllnessneedsto <br />betheprimary reasonfortheirpresenceinthefacility
