]Elk Ri verKA <br />Municipal U <br />Effective May 1, 2009 Elk River Municipal Utilities is required to collect and verify specific <br />identifying information from our customers. This is a result of the Federal Trade Commission <br />(FTC) regulations called "Red Flag Rules". Due to these regulations, we will need to have <br />identifying information on every person who should have access to information on the account. <br />These regulations were put in place to protect you, the customer, from having accounts set up in <br />your name without your authorization, or attempts to obtain information about you without your <br />authorization. Requiring your social security number on file (or a special password) helps us be <br />sure that we are providing your information to only you, or those you authorize. Once we have <br />your number, we won't ask for your full social security number when you call in, only the last <br />four numbers. If information provided doesn't match, we will not be able to provide any <br />information. <br />If we do not already have your social security number (or a special password) on file, we will <br />need to collect that and then have it verified when you are calling in. If only one person is listed <br />on the account, but others should be authorized, the person listed will have to call in and <br />authorize the additional person(s) (spouse, child, etc.) to receive information. We will also have <br />to collect identifying information on the additional person(s) being added to the account. <br />To help with the facilitation of collecting and updating this information, you may complete the <br />attached sheet and return it to our office for updating on your account. If you would rather call us <br />and provide the information over the phone, we would be happy to assist you in that way also. <br />Please be aware that if you do not provide us the information in the mail, or do not specifically <br />call in to provide it, we will have to collect the information from you when you call in for a <br />different reason and will not be able to proceed with providing any account information unless we <br />have the required identifying information.
