<br />AdmissionCriteria <br />AdmissionCriteria:Individualsconsideredforplacementmusthaveadiagnosisofaserious <br />andpersistentmental illnessby a Mental Health Professional.Noindividualisdiscriminated <br />againstinadmission, provisionofservices,ortermination becauseofrace, color,national <br />origin,religion, gender,physicalhandicap, sexualpreference,maritalstatus,orpublic <br />assistance status. <br />Applicantsseekingadmissiontotheresidential treatment/rehabilitationprogramwillmeet <br />following criteria: <br />some or allthe <br />1) IndividualhasamentalillnessasdefinedincomprehensivementalhealthactMINN. <br />Statues245.461. <br />Subd.20.(a): <br />2) Those on a provisional discharge orstay of commitmentwhohavethe potential of self-harm <br />maybeappropriateforthislevelofserviceiftheprogramdeterminesthisserviceprovides <br />adequatestructureandtherearesufficientsupport systemsinplaceinthecommunityand <br />safetyofothersisconsidered; <br />3)Individualcanreasonablybeexpectedtocommenceorresumeillnessmanagementandrecovery <br />skills/strategiesatthislevelofserviceandneeds24-hoursupervisedandmonitoredandfocused <br />treatment approachtoaccomplishthisimprovementinfunctioningandavoidrelapserequiringa <br />higherlevelofcare; <br />Individual symptoms are difficult to manage,orclearlyinappropriatefor,activetreatmentsatalesser <br />5) <br />levelofcare; <br />6) Individualneedsarestrictivesettingandisatriskofsignificantfunctionaldeteriorationifintensive <br />residential treatmentservicesarenotreceived. <br />7) Individualhasoneormoreofthefollowing: <br />•Ahistoryreoccurring or prolongedinpatienthospitalizationsinthepastyear <br />Significantindependentlivinginstability <br />•Homelessness <br />•Veryfrequentuse ofmentalhealthandrelatedservicesyieldingpooroutcomesinoutpatient <br />community supporttreatment <br />8) Individual,inthewrittenopinionofalicensedmentalhealthprofessionalandin consultationwith <br />thementalhealthcasemanager,hastheneedformentalhealthservicesthatcannot bemet with <br />or the individualislikelytoexperience amentalhealth <br />otheravailablecommunity-basedservices <br />crisisorrequireamorerestrictivesettingifintensive rehabilitativementalhealthservicesarenot <br />provided <br />9) Applicantsmustnotneedspecializeddietand/ornursing care. <br />10)Reviewofstaffingpatternsandcompetenciesofstaffresultinadeterminationthattheprogramcan <br />meettheneedsofapersonseekingadmission <br />Other Diagnostic Categories and Classifications we Will Accept that are Secondary to Mental Illness <br />1)We will take clients who have a secondary diagnosis of chemical dependency as long as they <br />are appropriate to the program. <br />2)We have taken people who have a secondary diagnosis of different personality disorders <br />depending on the client mix and our ability to work with them <br />3)We may take some people who have a mild TBI secondary to mental illness depending the <br />client’s ability to gain from the program. <br />4)According to policy at the Minnesota Department of Human Services we cannot take level 3 <br />sex offenders. We may take a level 1 or 2 if we believe the person will benefit from the <br />program and have a history of functioning well in the community. We try to consider the current <br />mix ofclients and the community they live in. It is important to note that mental Illness needs to <br />be the primary reason for their presence in the facility <br /> <br />
