Special Inspection Form (SIF). <br />Provide a completed and signed Special Inspection Testing and Inspection Schedule with <br />determined inspections in accordance with IBC Chapter 17. See Special Inspection requirements <br />listed on Sheet S1. The required third party inspection agencies must be contracted for said <br />work. Plan review cannot be completed and a building permit cannot be issued until a completed <br />SIF is submitted, reviewed and approved. [MR 1300.0130, Subp. 1] <br /> <br /> Signatures are required (where applicable) for the Owner, Contractor, Architect, <br />Structural Engineer of Record, Special Inspector (structural), Testing Agency, Special <br />Inspector (technical) and Fabricator. <br /> Where fabrication of structural load-bearing members and assemblies is being performed <br />on the premises of a fabricator’s shop (structural steel), special inspection of the <br />fabricated items shall be required. Exception: the fabricator is registered and approved to <br />perform such work without special inspection; provide documentation for review (copy of <br />AISC Certification). Copies of all required third party inspection reports and/or <br />observation logs must be submitted to the Building Official as soon as they are completed <br />for code review and record purposes. [IBC 1704.2.5.2] <br />o If the fabricator is not AISC certified, the SIF will need to be signed by a third <br />party testing agency that has been retained to provide Special Inspection on-site in <br />the fabricator’s shop (provide documentation). <br /> <br />2. Specification. <br />Is there a Specification book for this project? Please provide a copy. Please address. <br /> <br />3. Structural Calculations. <br />As part of the plan review process, certified sample structural calculations must be submitted for <br />the required structural review for this project. Sample structural calculations are to include the <br />title and year of the codes used, live loads, dead loads, wind loads on the primary system, wind <br />loads on the components, a sample of each type of lateral load resisting element, a sample of <br />each vertical load resisting element, and a sample of any special condition (and any specific <br />requested calculations, see Structural Comments below). Calculations are to be presented in a <br />clear, legible and organized manner conductive to a structural plan review. A table of contents <br />should be provided and pages are to be numbered for referencing purposes.
