ORDINANCE 94- 677 <br /> • Page 6 <br /> Clerk of District Court shall be attached to the <br /> application. <br /> m. If the applicant is a corporation or other <br /> organization, the applicant shall submit the following: <br /> (1) Name, and if incorporated, the state of <br /> incorporation; <br /> (2) A true copy of the certificate of <br /> incorporation, articles of incorporation or <br /> association agreements; <br /> (3) The name of the manager or proprietor or other <br /> agent in charge of, or to be in charge of the <br /> premises to be licensed, giving all information <br /> about said person as is required in the case of a <br /> single applicant. <br /> (4) A list of all persons who, single or together <br /> with their spouse own or control an interest in <br /> said corporation or association in excess of five <br /> i percent or who are officers of said corporation or <br /> association, together with their addresses and all <br /> information as is required for a single applicant. <br /> n. The amount of the investment that the applicant has <br /> in the business, land, building, premises, fixtures, <br /> furniture, or stock in trade, and proof of the source of <br /> the money. <br /> o. A list of responsible persons, including the names <br /> of owners, managers and assistant managers, who may be <br /> notified or contacted by State or City employees in case <br /> of emergency. These persons shall be residents of the <br /> Twin Cities metropolitan area as defined by 'Minnesota <br /> Statute, Section 473 . 121, Subdivision 2 . <br /> p. Whether the applicant holds a current pawnbroker or <br /> precious metal dealer license from any other governmental <br /> unit and whether the applicant is licensed under <br /> Minnesota Statutes, Section 471. 924 or 325F.731 to <br /> 325F.744 . <br /> q. Whether the applicant has ever been denied a <br /> pawnbroker or precious metal dealer license from any <br /> • other governmental unit. <br /> r. The location of the business premises. <br />
