<br />. <br /> <br />. <br /> <br />. <br /> <br /> <br />GENERAL BENEFITS <br /> <br />This is a brief description of benefits available to eligible employees. The descriptions provided <br />here are not intended to be comprehensive and all questions regarding eligibility and benefit levels <br />should be directed to your supervisor so the specific plan documents can be reviewed for an <br />answer. The plan documents govern any inconsistencies between these documents and the <br />information provided here. Benefits and eligibility requirements are subject to change, and such <br />changes may not be reflected in this description. <br /> <br />The City expects to offer its benefit plans for the foreseeable future, but it reserves the right, in its <br />sole discretion, to change, modifY or eliminate them at any time, except to the extent prohibited <br />by law. <br /> <br />INSURANCES <br /> <br />Various insurance policies, including group medical insurance, may be available for eligible City <br />employees and their dependents. The eligibility requirements and benefits provided are specified <br />in materials provided by the respective insurance carriers. The respective carriers may have <br />different requirements concerning the eligibility of employees. The requirements of the respective <br />carrier will govern eligibility. <br /> <br />Under some circumstances, eligible employees and their dependents may have the option of <br />continuing coverage under certain City employee insurance plans for a limited time at the <br />employee's own expense when coverage would otherwise end. This continuation right is <br />provided in accordance with COBRA (Consolidated Omnibus Budget Reconstruction Act of <br />1985) and applicable state law. Contact the Finance Department for additional information. <br /> <br />DEFERRED COMPENSATION <br /> <br />The City provides eligible employees the opportunity to participate in a Deferred Compensation <br />Plan. This is a voluntary plan that allows employees to place a portion of their earnings into a <br />pre-tax deferred investment program. Contact the Finance Department for information on this <br />plan. <br /> <br />FLEXffiLE BENEFITS PLAN (Section 125) <br /> <br />The City offers a plan in which a portion of an eligible employee's pay can be set aside, pre-tax, to <br />cover certain estimated health care and day care costs. This contribution is deducted from gross <br />wages and is reimbursed to the employee upon proof of the medical or day care expenses. The <br />employee must expend all the money contributed to the flexible benefit plan or lose it. For <br />additional information on this plan, contact the Finance Department. <br /> <br />19 <br />
