RESOLUTION 04- <br /> <br /> A RESOLUTION FOR THE CITY OF ELK RIVER <br /> <br />RESOLUTION APPOINTING A CONTACT DEPARTMENT AND <br /> ASSIGNING DUTIES <br /> <br />WHEREAS regulations of the Health Insurance Portability and Accountability Act of <br />1996 (HIPAA), 45 CFR 164.530 (a), require the City of Elk River appoint one <br />department as the contact department. <br /> <br />· to receive complaints about any failure to comply with either the <br /> policies and procedures or the privacy rules in general; and, <br /> <br />WHEREAS the Elk River City Council shares concerns over privacy of health data <br />protected by the Act and wishes to comply with the requirements of the law, <br /> <br />NOW, THEREFORE BE IT RESOLVED, the City Council of the City of Elk River, <br />Minnesota, hereby appoints the Finance Department as the contact department. <br /> <br />Passed and adopted this 12th day of April 2004. <br /> <br />ATTEST: <br /> <br />Stephanie A.
