employment ends involuntarily because of lack of work, will have 50% of <br /> unused sick leave, up to a maximum of 120 days, converted into cash and <br /> deposited into their Post Employment Health Care Savings account provided <br /> they sign and do not rescind an agreement releasing claims arising out of their <br /> employment, in a form prescribed by the Utilities. Employees involuntarily <br /> terminated by the Utilities for any reason other than lack of work shall not be <br /> eligible to receive such conversion. No contributions will be accepted by the <br /> Plan on behalf of an employee after the death of the employee. <br /> 35:40. 457 DEFERRED COMPENSATION <br /> The Utilities will match funds contributed by employees, up to 3% of their base salary <br /> annually,with a maximum contribution of$2,000.00. These plans are administered by the <br /> Minnesota State Deferred Compensation Plan and/or Wenzel & Associates' ING Plan. <br /> The Utilities' contribution will be made May 31st of each plan year for the employee. If an <br /> employee departs prior to May 31st, she/he will receive a prorated contribution for that <br /> year. For a complete plan description consult the summary plan description distributed to <br /> each employee or available upon request. <br /> 49A1. HEALTH INSURANCE COVERAGE <br /> For eligible employees who regularly work 30 hours per week, and enroll in thea health <br /> insurance plan provided by the Utilities, the Utilities will pay eightyseventy-five percent <br /> (5075%)of the monthly premium per employee/family for health insurance. fey <br /> premium over and above SOThe Utilities currently offers both a conventional plan and a <br /> Health Savin's Account-related elan. The amount of the monthly premium for a health insurance plan over and above 75% <br /> of the total monthly premium for health care coverage is the responsibility of the employee. <br /> Employees should consult the Summary Plan DesetiptienDescriptions of the available <br /> plans for details regarding coverage and eligibility. A copy of these <br /> descriptions will be provided to employees and is available upon request. <br /> 40:42. DENTAL INSURANCE <br /> Eligible employees who regularly work at least 30 hours per week and who enroll in the <br /> Utilities' dental insurance plan, the Utilities will pay seventy-five percent (75%) of the <br /> monthly premium per employee/family for dental insurance. Any monthly premium over <br /> and above 75% of the dental coverage is the responsibility of the employee. Employees <br /> should consult the Summary Plan Description for details regarding coverage and <br /> eligibility.
