Name of Applicant: ( please print) Applicant's Address: Phone: Specific Location of Block Party (clearly identify what portion of the street is to be barricaded): (street) Date of Activity: NOTE: between and (cross street) (cross street) Time: From to NO PERMITS WILL BE ISSUED FOR BLOCK PARTIES LASTING AFTER ONE- HALF HOUR BEFORE SUNSET o o PROVISIONS Applications shall be submitted to the City Clerk on forms provided and shall contain the name and other pertinent information of the adult person(s) responsible for the party, together with signatures of at least sixty (60) percent of the adult residents of the affected block. No permit shall be issued which does not contain the above required number of signatures. Block parties shall be restricted to residents of the affected block and their invited guests. The applicant must make a good-faith attempt to deliver to each residence or building management or tenants association abutting such portion of the street a notice containing the following information: (a) (b) (c) (d) A description of the party; The date and hours of the party; The name, address and phone number of the sponsor; and That anyone objecting to the block party may petition the City Council for a hearing to protest the issuance of a permit for such party.
