Sherburne County � � <br /> PUBLIC WORKS DEPARMEArr <br /> 425 Jackson Ave N <br /> Elk River, MN 55330 <br /> (763) 765-3354 <br /> Fax: (763) 765-3366 <br /> April 15, 2013 <br /> Tom Dickson <br /> Northern Natural Gas <br /> 1120 Centre Pointe Drive; #400 <br /> Mendota Heights, MN 55120 <br /> Re: Commercial Access Permit No. 13-33-02; <br /> Dear Mr. Dickson, <br /> The access permit that you applied for, to County State Aid Highway 33 (CSAH 33), has <br /> been approved. <br /> The permit is approved with the following requirements: <br /> • The access top will be a width shown on your plan (approximately 118ft) and the <br /> side slopes will be 6h:1v or a horizontal distance of 24ft. The access will drain <br /> away from the roadway. <br /> • A MnDOT standard plate 3126 culvert (15" CMP) with aprons and grates are <br /> required. <br /> • All disturbed areas within the right of way must be restored with seeded topsoil <br /> and mulched to obtain a 70% uniform distribution of re-established grass. <br /> • All traffic control procedures and devices must conform to the Minnesota Manual <br /> on Uniform Traffic Control and be set in place and maintained by your contractor. <br /> Enclosed is the original Access Permit Application. Please carefully read the <br /> requirements on the backside of the permit and standard profile detail sheet that <br /> was given to you at our site visit. After the grass is re-established to the requirement <br /> mentioned above and on the permit, date and sign the bottom portion of the application <br /> and return the entire sheet to the Sherburne County Public Works to initiate a final <br /> inspection. Upon final inspection and approval the $1000.00 Access Bond Fee will be <br /> returned to you.
