REQUIRE,MENTS TO MEET THE CONDITIONS OF THE PERMIT <br /> 1. No work under this application is to be started until application is approved and the permit granted. <br /> 2. if mail delivery service is necessary, Sherburne County requires an approved Swing-Away Mailbox Support(MNDOT Standard plate 9350A) <br /> will be installed by SCPWD and included as part of the permit fee.Notify the permit agent after the location of placement is determined. <br /> 3. Where work on the traveled roadway is necessary,traffic must be protected in accordance with the standards listed in the MN MUTCD manual. <br /> 4. Neither supplies nor excavated materials(dirt,sod,gravel,bituminous,etc.)shall be deposited on the County Highway Right-of-Way during the <br /> construction of driveway or installation of drainage facilities. <br /> 5. Access till slopes shall be constructed at a 6:1 within the ROW and shall be finished with 4" of topsoil, grass seeded and mulched. <br /> Seventy percent uniform distribution minimum re-establishment of turf and any erosion repairs are required, prior to final approval. <br /> Horizontal slope lengths for a 3'ditch(is 3'X6')= 18ft.;4'ditch(4'X6')=24ft.;5'ditch(5'X6')=3011.(See attached detail sheet) <br /> 6. Driveways shall be so constructed as to slope down and away from the shoulder line of the County Highway for a distance of 10 feet. <br /> (See attached detail sheet) <br /> 7. All culverts placed in out-ROW shall meet Sherburne County requirements and culvert ends shall have 6:1 safety aprons and grates. Culverts <br /> shall be provided and installed by the applicant/developer.(See attached detail sheet)SCPWD requires RCP culverts under public roads. <br /> 8. Access removals must match existing shoulder,down slope of the road,and ditch grade on either side of removed access. <br /> (See attached detail sheet) <br /> 9. Once completed and accepted any changes or alterations to the access within the ROW will require a new permit. <br /> 10. After driveway construction is completed the permittee shall complete the bottom portion on the other side of this form and return a copy of the <br /> permit to Sherburne County Public Works for final inspection and approval. <br /> 11. The Access Bond Fee shall be returned to the applicant only when all of the required conditions of the permit have been satisfied. If the <br /> access construction is not completed per the requirements listed above,the permit will be considered a Right of Way Violation and any <br /> costs incurred by the County to complete the unfinished construction shall be charged to the applicant. <br /> GENERAL INFORMATION <br /> By MN State Statutes,no boulders,landscape timbers,retaining walls,plantings,fence posts and wire,etc. are permitted <br /> within road right of ways. <br /> ➢ Sherburne County reserves the right to control the access point to the County highway and may require that it be shared. The <br /> access point is determined with consideration to the following factors: meet the spacing requirements in the Transportation <br /> Plan, one access per parcel, avoid or minimize wetland impacts, safest location on the County highway relative to vertical <br /> and horizontal curves. <br /> The applicant is required to submit a drawing(with the completed permit application)of the property indicating the proposed <br /> access location or meet with the Permit Agent on site. The applicant should stake the proposed desired access point. <br /> All accesses, residential, commercial and industrial entrances shall be constructed per MnDOT standard plate no. 9000D <br /> regarding access width and radius. Consideration for special situations may be approved by the County Engineer on a case <br /> by case basis. <br /> ➢ To minimize site plan changes, it is desirable that preliminary driveway layouts for proposed driveways at commercial sites <br /> be submitted by the applicant to the County Engineer prior to making the permit application. By this means, construction and <br /> maintenance plans for the portion of the county highway under consideration may suggest alternate or improved methods or <br /> standards of construction or reconstruction to the property owner and/or applicant. <br /> > Bituminous or concrete paving of a driveway does not require a permit,if there are no other alterations. <br /> > An approved application grants permission for the construction of the access in accordance to the requirements listed above, <br /> any accompanying standard detail sheet(s)and any special provisions outlined in the supplemental letter. <br /> Rev. 3/26/13 <br />
