(3) The cost of health insurance provided by the Company (or its tenants in <br />the Project) for the above-referenced jobs, separated by bands of wages, is as follows: <br /> <br />Wage Level Per Hour <br /> <br />Cost of Health Insurance <br /> <br /> (4) If the Company has not already met the Job Goals, it reasonably expects <br />that it will meet those goals on or before ,200__, and is taking the <br />following steps to meet the Job Goals: <br /> <br /> (5) The Company's completion of the Project did not involve the relocation by <br />the Company of any of its facilities located elsewhere. <br /> <br />(6) The Company has no parent corporations. <br /> <br /> (7) Other than the assistance and subsidy provided by the City under the <br />Agreement, there are no other State of Minnesota or "local government agency" grants of <br />business subsidy to the Company for the Project, except for: <br /> <br />Grantor Value ($) <br /> <br />Grantor Value ($) <br /> <br /> (8) The Company hereby agrees to provide upon request such other <br />information as the Commissioner of the Department of Trade and Economic <br />Development of the State of Minnesota may request the City or the Company to provide <br />or as may be required by the Subsidy Law. <br /> <br /> (9) The Company represents that it has continuously operated and leased the <br />Project since its completion and expects to continue to do so for the foreseeable future. <br /> <br />* Would add Sherburne County property tax abatement here ($200,000 requested), if granted. <br /> <br />1410047vl A~2 <br /> <br /> <br />
